What services can I expect from my Care Advisor?
Our goal is to provide you and your family with resources and care recommendations to assist you in finding the best possible care that fits your needs. We do this by teaming up with you to identify goals and create personalized action plans to track your interests, needs, and actions. Our Care Advisors will work directly with you, developing a thorough understanding of your goals, needs, and concerns and providing support along the way.
What is a personalized action plan?
A personalized action plan is a document made specifically for you. The plan will be a roadmap of sorts, outlining your Care Advisor’s recommendations for how to best meet your needs, helping you live independently for as long as possible. Personalized action plans typically consist of resources and local recommendations from your Care Advisor.
